Iðjumatstækið ADL-focused Occupation-based Neurobehavioral Evaluation (A-ONE) er staðlað markbundið matstæki notað til að meta samtímis framkvæmdafærni við daglegar athafnir og áhrif taugaeinkenna á framkvæmdina. Markmið þjálfunarnámskeiðs fyrir notkun A-ONE er að kenna iðjuþjálfum áreiðanlega notkun matstækisins. Fjallað verður um fræðilegan bakgrunn A-ONE, klíniska rökleiðslu, fyrirlögn, úrvinnslu upplýsinga og stigagjöf.

Námskeiðið er kennt á ensku.

The goal of the ADL-focused Occupation-based Neurobehavioral Evaluation (A-ONE): Training course for administration and scoring is to train occupational therapists in reliable use of the A-ONE evaluation through education on the theoretical background, clinical reasoning involved, administration and scoring procedures of the instrument.
The A-ONE, a standardized criterion-based instrument, is used by occupational therapists to assess persons with neurological disorders affecting activities of daily living (ADL).

The A-ONE enables occupational therapists to determine the level of assistance needed for performing ADL and simultaneously determine the underlying reason for the lack of independence. In particular, it aids in determining the impact of neurobehavioral impairments on ADL task performance in natural contexts. That is, it simultaneously provides information on the level of ADL performance (dressing, grooming/hygiene, transfers/mobility, feeding, functional communication) and impairments (i.e. motor apraxia, ideational apraxia, unilateral body neglect, unilateral spatial neglect) impacting the level of independence. 

When using the A-ONE, determination of the type of impairment(s) impacting observed ADL performance resulting in occupational performance errors is based on hypothesis testing through specific clinical reasoning procedures. 
The A-ONE consists of two ordinal rating scales, a Functional Independence (FI) scale focusing on ADL performance and a Neurobehavioral impairment (NB) scale focusing on type of impairment and severity of impact on ADL.
Research background of the A-ONE includes published articles on both Classical test theory studies and Modern test theory studies with application of Rasch analysis.

Occupational therapists report that the A-ONE provides profession-specific information on task performance and type of limitations. This information has been valuable to occupational therapists and other professional team members. The evaluation can also be used to obtain information to aid therapists’ decision making and setting goals. It can further assist therapists in choosing intervention methods.

Five-day training courses to practice clinical reasoning are necessary for reliable administration and scoring of the evaluation.

The A-ONE, created in 1987, is claimed to be the first published evaluation on functional cognition in the world and to still be the only one used to evaluate impact of a wide spectrum of neurological impairments on task performance.


Topics covered 
 

  • The conceptual background of the A-ONE will be outlined as well as definitions of concepts used in the evaluation.
  • Evaluation of cognitive, perceptual and motor dysfunction from a functional viewpoint so that the effects of neurobehavioral deficits on task performance can be studied.
  • Occupational performance and use of reasoning skills for detecting performance errors and relating the errors to dysfunction of the central nervous system (CNS).
  • Differentiation between different neurobehavioral impairments.
  • Different patterns of impairments due to CNS disorders.
  • Administration and complex scoring procedures of the A-ONE instrument.
  • Validity and reliability of the evaluation.
  • Diverse utilization possibilities of the A-ONE and its conceptual background in addition to the standardized evaluation, these including hypotheses testing and measurement potential.


What you will learn

  • Ability to evaluate ADL task performance and performance errors and to use clinical reasoning for relating neurobehavioral errors to the underlying cognitive, perceptual and motor impairments causing diminished performance in persons with neurological disorders.
  • Ability to develop clinical reasoning strategies based on operational definitions and knowledge of neurological processing to differentiate between the various presentations of the impairments.
  • Proficiency in the A-ONE administration and scoring procedures, and ability to use it with increased reliability.
  • Participants will become certified in the use of the A-ONE after successful completion of the course. 


Who should attend? 


Certified occupational therapists that work with neurological clients and are interested in assessment of functional cognition and application of combined performance and task analyses to evaluate impact of neurological impairments on ADL performance.

 

Information:

Upon registration each participant will be sent reading materials. Participants are strongly encouraged to read the materials prior to the course for maximum benefits. A pre-course assignment based on the reading materials is expected to be handed in on the first day of the course. The course format includes presentations, case studies, discussions, as well as practical reading assignments and exercises. Note that homework assignments are included in the course, and there is a specific final assignment that requires preparation. This is an “open book” assignment. Inbound lecture notes will be provided during the course as well as evaluation forms. Icelandic participants are encouraged to review the publication “A-ONE: Fræðilegur bakgrunnur og iðjumiðað námsefni” before the course.

Teachers:

Guðrún Árnadóttir, PhD is the developer of the A-ONE instrument and author of the book The Brain and Behavior: Assessing Cortical Dysfunction through Activities of Daily Living. Additionally, she has authored several published book chapters (the best known in all five editions of Stroke Rehabilitation: A Function Based Approach) and peer reviewed journal articles. Her work in occupational therapy includes clinical work, research, as well as developing, teaching and coordinating numerous A-ONE training courses in different countries and continents for over 30 years. Her present position within the department of occupational therapy at Landspítali, The National Hospital in Iceland, is focused on professional development and research. She is also an Associate professor of clinical occupational therapy at Faculty of Medicine, University of Iceland. She studied occupational therapy in Manitoba Canada at an undergraduate level, obtained her Master’s degree in occupational therapy at University of Southern California in Los Angeles, USA and a doctoral degree in Occupational therapy from Umeå University in Sweden. She has received several grants and awards during her carrier including the “The American Occupational Therapy Association Recognition of Achievement Award for” “Recognition of contributions as THEORIST, RESEARCHER, EDUCATOR, CLINICIAN, ACTIVIST and ACADEMIC WRITER“.

Valerie Harris, MSc is an occupational therapist who currently works as the manager of outpatient rehabilitation Kjarkur rehabilitation center in Iceland. She has previously held the position of lecturer in occupational therapy in the undergraduate program at the University of Akureyri, Iceland and at La Trobe University, Victoria, Australia. She is currently a part time lecturer at the University of Akureyri. Valerie completed the A-ONE trainer program in 2013 and has since participated in teaching the A-ONE training courses in Asia, USA and Europe. She completed her undergraduate studies at La Trobe University and obtained her Master’s degree from Florida International University, USA. Valerie’s main areas of interest are rehabilitation of adults with neurological problems, support for carers and clinical reasoning.
